You and Your General Practice (YYGP) has been developed to help patients understand what to expect from their general practice and how they can get the best from their GP team. YYGP also enables patient to provide feedback or raise concerns with their GP Practice, Healthwatch or the integrated care board (ICB). The YYGP document published by NHS England can be found below.

Veteran Friendly Accreditation
We are incredibly proud to be accredited as a Veteran Friendly GP Surgery. We have a member of staff in our PCN who is our Veteran Support Link - please ask for Emily Ind if you need any support. The following "Flip Book" is a fantastic reference guide which includes lots of support groups and information for any Veteran. In time of bereavement
In the unfortunate event that a person has passed away, there are a number of things that need to be done.
Autism Hospital Passport
This Hospital Passport contains the contact details for the LD / Autism Liaison service for Great Western Hospital, so that autistic people can seek support from a department within the hospital.
